Se ha demostrado que las PGs E tienen efectos anabólicos en el tejido óseo y que la PGE₂ estimula la síntesis del calcitriol in vitro, pero esto no ha sido investigado con la PGE₁. Estudios de fracturas palatinas en gatos y conejos en nuestro laboratorio, indican que la PGE₁ reduce significativamente el tiempo de mineralización ósea, lo que sugiere que la aceleración de la formación ósea se debe a un aumento en la producción de calcitriol. El objetivo del presente estudio fue el determinar tanto in vivo como in vitro, los efectos de la PGE1 sobre la síntesis del calcitriol Se utilizaron 20 conejos adultos machos de la cepa Nueva Zelanda, dividiéndolos en dos grupos; diez recibieron diariamente por vía intravenosa el vehículo durante 20 días y los otros diez recibieron diariamente por la misma vía 50 µg de PGE₁ en el vehículo durante 20 días. El día 20, se obtuvieron muestras de sangre donde se determinaron los niveles séricos de calcio total e iónico, magnesio total e iónico, fósforo, fosfatasa alcalina total y su isoenzima ósea, hormona paratiroidea (PTHi), calcitonina, 25-hidroxivitamina D₃ (25(OH)D₃) y 1,25-dihidroxivitamina D3. Para el estudio in vitro se obtuvieron diez riñones de cinco conejos mantenidos en condiciones iguales a los utilizados en el experimento in vivo. Se preparó una solución enriquecida con los túbulos renales proximales (TRP) mediante el método de gradiente de Percoll. En el cultivo de la solución de TRP se hicieron dos estudios cinéticos del tipo dosis-respuesta: el primero fue para buscar la concentración adecuada de 25(OH)D₃ a emplearse como sustrato en la producción de calcitriol, en el segundo se buscó la concentración adecuada de PGE1 a emplearse como estimuladora para la producción de calcitriol. Se realizó un tercer experimento, utilizando cuatro viales que contenían TRP, los cuales se sometieron a pre-incubación por 20 minutos. Después se inició la reacción agregando a todos los viales 8 µM de 25(OH)D₃, como sustrato. A los viales II, III y IV, se les agregó una concentración de PGE₁ de 2.82 x 10¯⁶ M y a los viales III y IV, una concentración de 10¯⁴M de AH6809 (antagonista del receptor de las PGs). En los viales I, II y III la reacción se suspendió a los 5 minutos y en el vial IV a los 30 minutos, adicionándoles 1 mL de acetonitrilo. La producción de calcitriol se midió por el método de radioinmunoanálisis empleando I125 como marcador y expresando los resultados en pg/mg de proteína a los 5 y a los 30 minutos, respectivamente. En el experimento in vivo, los niveles séricos de calcio y magnesio fueron significativamente elevados en el grupo que recibió PGE1 cuando se les compara con el grupo que recibió vehículo. Los niveles séricos de hormona paratiroidea (PTHi), 25(OH)D₃ y calcitonina fueron semejantes en los dos grupos, pero se observó un incremento significativo en los niveles séricos de calcitriol (p< 0.01) en el grupo que recibió PGE1. En el experimento in vitro, los resultados muestran que existe un incremento significativo (p< 0.04) de la síntesis de calcitriol en presencia de PGE₁, mientras que el AH6809 en presencia de PGE1 a los 30 minutos disminuyó significativamente (p< 0.05) la síntesis de calcitriol hasta niveles basales. El estudio demuestra claramente que tanto in vivo como in vitro la PGE₁ incrementa la síntesis del calcitriol. El incremento de la síntesis del calcitriol con PGE1 es similar al que ha sido reportado con PGE₂. El antagonista del receptor de las PGs (AH6809) bloquea la síntesis del calcitriol al estímulo con PGE₁.; It is well known that prostaglandins are potent regulators of bone metabolism. Prostaglandins of the E serie stimulate bone resorption and have anabolic effects on bone formation. Previously reports indicate that prostaglandin E2 increases calcitriol synthesis in vitro. However the role of prostaglandin E1 in the bone metabolism has not been investigated. The fact of PGE1 treatment reduces bone mineralization lag time on palatine bone fractures in cats and rabbits, suggests an increase in calcitriol synthesis produced by the PGE1. The aim of this study was to determine the in vivo and in vitro effect of PGE1 on calcitriol synthesis. For in vivo studies, daily intravenous injections of vehicle or prostaglandin E1 (50 µg) were applied to two different groups of rabbits. Twenty days later, serum samples were taken to measure total and ionic calcium, total and ionic magnesium, phosphorus, total and bone alkaline phosphatases, parathyroid hormone, calcitonin, 25 hydroxyvitaminD3 and 1,25 dihydroxyvitaminD3 levels. For in vitro studies, the kidneys of the rabbits were removed and an enriched preparation of renal proximal tubes (RPT), using the Percoll gradient method, was prepared. Using RPT, two in vitro studies were carried out in order to obtain adequate doses of 25(OH)D3 to be used as substrate and PGE1 as stimulator, to produce optimal amounts of calcitriol. A third in vitro experiment was carried out with four vials containing PRT that were submitted to preincubation for 20 minutes. After the reaction was started, we added to all the vials 8 µM of 25(OH)D3, and to vials II, III and IV: 2.82 x 10-6 M of PGE1 and to vials III and IV: 10-4M AH6809 (receptor PGs antagonist ). In vials I, II and III, the reaction was suspended at 5 min and in vial IV at 30 min, adding 1 mL acetonitrile. Calcitriol production was measured by radioimmunoassay employing I125 as tracer. The results showed that serum levels of Ca, Mg and P were increased in the group treated with daily injections of PGE1. Serum levels of PTHi, 25(OH)D3 and calcitonin remained similar to those determined in the control group, however a significant increase in calcitriol serum levels was observed. On the other hand, in vitro experiments showed that PGE1 increased significantly calcitriol synthesis. After 30 min of incubation in the presence of AH6809, 25(OH)D3 and PGE1, the RPT decreased significantly the synthesis of calcitriol, until basal levels. The present study shows clearly that PGE1 increases the synthesis of calcitriol in vivo and in vitro. The increase of calcitriol synthesis observed with PGE1, is similar to that reported for PGE2.
